These Website Terms & Conditions ("Terms") govern access to and use of womenshormonehealthandwellness.com and related public webpages, forms, quizzes, downloads and online content operated by Janna Block Wellness LLC, doing business as Women's Hormone Health & Wellness ("WHHW," "we," "us," or "our"). By using the website, you agree to these Terms. If you do not agree, do not use the website.
1. Eligibility
The website, public forms, Hormone Quiz, webinars and free resources are intended for adults age 18 or older. Clinical services are not offered to anyone younger than 18.
1A. Geographic availability and licensure
Public website content and educational resources are available to people regardless of location. Clinical services are available only when and where Janna Block holds an active professional license that authorizes the care, currently Minnesota. Arizona and Florida are not current clinical-service locations and may be added only if and when the applicable license actually issues.
2. No provider–patient relationship
Using the website, submitting a public form, taking the Hormone Quiz, downloading a resource, registering for a webinar or participating in a Hot Hormone Hustle does not create a provider–patient relationship with Janna Block or WHHW.
WHHW intends to accept a person as a clinical patient only after Janna approves the person for care, required intake and consent documents are completed and signed, and required payment is made. Actual legal relationship timing may also depend on the parties' conduct and applicable law.
3. Educational information only
Website content, social content, quizzes, webinars, guides and other public materials are provided for general educational and informational purposes. They are not diagnosis, treatment, medical advice, prescribing, laboratory interpretation or a substitute for care from a qualified healthcare professional who knows your circumstances.
Do not start, stop or change medication, hormones, supplements, testing or treatment based only on public WHHW content. Maintain an appropriate primary-care provider and seek individualized care when needed. The Medical Disclaimer is incorporated into these Terms.
4. Emergencies and urgent needs
WHHW is not an emergency service and does not replace primary, urgent or emergency care. Do not use the website, forms, email, text messaging, patient portal or WHHW messaging channels for emergencies. Call 911 or seek appropriate urgent or emergency care when necessary.
5. Hormone Quiz
The Hormone Quiz generates an educational result based on submitted responses. A result may describe symptoms as potentially aligning with a general pattern, but it does not diagnose a condition, establish medical necessity or determine treatment. Results may be incomplete or inaccurate and must not be treated as clinical findings.
Under the current configuration, WHHW does not retain individual symptom selections submitted through the Hormone Quiz. If you authorize marketing follow-up, WHHW may temporarily use a general pattern classification to route related educational communications. That classification is not a diagnosis and is removed after its operational purpose is complete. WHHW may retain limited contact, consent, refusal, revocation and opt-out records as reasonably necessary for communication, compliance and legal purposes. 6. Hot Hormone Hustle
The Hot Hormone Hustle is a free, nonclinical introductory and fit conversation. It is not a medical evaluation, chart review, diagnosis, quiz interpretation, treatment plan, prescribing visit or promise that WHHW will accept you for clinical care.
7. Clinical services and separate agreements
Clinical services are cash-pay. WHHW does not bill insurance and does not provide superbills. At a patient's request, WHHW may provide a basic reimbursement letter for the patient to submit to an insurer or benefit plan. WHHW does not submit the letter directly, does not guarantee coverage or reimbursement, and does not represent that the letter satisfies a plan's claim requirements. Prices, scope, scheduling, payment, cancellation, card-on-file, refund and clinical terms are stated at enrollment or checkout and in the applicable patient or service agreement.
If these Terms conflict with a signed consent, financial policy, card authorization or service-specific agreement, the more specific signed document controls for that clinical service to the extent allowed by law.
7A. Privacy and patient health records
WHHW is a Minnesota health-care provider subject to the Minnesota Health Records Act and other applicable privacy, professional and data-protection laws. WHHW does not currently conduct the HIPAA-standard electronic transactions that would make it a covered entity under the federal Health Insurance Portability and Accountability Act. Nevertheless, as a matter of policy and patient trust, WHHW voluntarily applies relevant privacy, security and breach-response safeguards informed by HIPAA as an internal minimum operating standard. Where Minnesota or another applicable law provides greater protection, the more protective requirement controls. Additional information concerning WHHW's health-record practices and patient rights is provided in WHHW's Notice of Privacy and Patient Health Record Practices.
Acceptance of these Terms does not constitute a blanket authorization for WHHW to disclose a patient's health records and does not waive any nonwaivable right provided by Minnesota or federal law. Any disclosure requiring patient authorization requires a separate, signed and dated authorization that satisfies applicable law. Website use, program enrollment, payment and agreement to these Terms do not by themselves authorize WHHW to release patient health records.
7B. Clinical technology, AI assistance and recordings
WHHW may use Practice Better as its patient portal and clinical record system; Practice Better or another configured system for clinical messaging; Zoom for telehealth; Heidi AI to capture visit audio, create transcripts and assist with draft clinical notes; and Fathom to record and transcribe telehealth visits. A specific tool is used only as described in the applicable patient notice and consent.
AI-generated transcripts, summaries and draft notes may contain errors. Janna remains responsible for clinical judgment and reviews or edits the clinical note before relying on it. Recording or transcription consent is separate from website use, marketing permission and payment authorization and may be withdrawn prospectively as stated in the clinical consent.
7C. Medicare beneficiaries and cash-pay services
WHHW does not participate in, accept payment from, or bill Original Medicare or Medicare Advantage, and WHHW does not submit Medicare claims or provide Medicare superbills. Medicare beneficiary status must be disclosed before any paid clinical agreement is signed, appointment is booked, or payment is accepted.
A person who is enrolled in Original Medicare or Medicare Advantage, or who is unsure, may not use WHHW's standard clinical enrollment, booking, agreement or checkout process. WHHW will not accept payment or begin paid clinical services through that process. The person must contact WHHW for review.
Unless and until Janna completes every federal requirement applicable to private cash-pay care of a Medicare beneficiary, including any required opt-out affidavit and beneficiary-specific private contract, WHHW will not accept that person for a service that is or may be Medicare-covered. A Website Term, CALM Reset agreement, standalone-service agreement, card authorization or generic cash-pay waiver does not replace a Medicare private contract. WHHW does not promise future acceptance of a Medicare beneficiary.
8. Payments and failed payments
You agree to provide accurate payment information and pay charges you authorize. Payment in full is preferred. WHHW may approve an installment plan or individualized payment arrangement in writing. The applicable checkout disclosure, payment authorization and service agreement state the required down payment, remaining amounts, due dates and authorized payment method. No automatic or discretionary charge may be made without the authorization required by the applicable agreement and law.
If an amount becomes overdue, WHHW may apply the consequences stated in the applicable service agreement, including postponing nonurgent services until the balance is paid or WHHW approves a written arrangement. The service agreement disclosed before enrollment controls whether a payment-related postponement is treated as a missed or used visit and whether any discretionary makeup visit is offered. Any termination of an established clinical relationship remains subject to applicable notice, continuity-of-care and professional obligations.
9. Refunds
Refund and cancellation terms for each clinical service are stated in the applicable service agreement and disclosed before purchase or enrollment. Those service-specific terms control for that clinical service, subject to applicable law.
Unless different refund terms are displayed before purchase, paid digital products, downloadable resources, recorded or live classes and webinars, and access to WHHW software or mobile applications may be refunded before the digital product or access is delivered, activated or made available, or before the live event begins. No refund is available after the digital product or access is delivered, activated or made available, or after the live event begins, except where required by law.
10. Scheduling and messaging
For purposes of these Terms, 'Normal Business Hours' means Monday through Friday, 9:00 a.m. to 5:00 p.m. Central Time. Janna may choose to answer calls or messages or schedule services outside Normal Business Hours at her discretion. Doing so does not change this definition, promise future availability or create continuous monitoring.
Unless a separate written service agreement states a more protective rule for the patient, at least 48 hours' notice is required to cancel or reschedule a clinical appointment. An appointment cancelled with less than 48 hours' notice or missed without notice may be non-refundable, treated as used when prepaid, or charged at the full scheduled rate when booked separately, as disclosed before booking or enrollment. The 48-hour rule applies to appointments booked on or after September 1, 2026; an appointment booked earlier remains governed by the cancellation terms disclosed or agreed to when that appointment was booked. A late arrival does not extend the scheduled appointment, and WHHW may shorten, reschedule or treat the appointment as missed if insufficient time remains for safe and appropriate service. WHHW may make exceptions or offer a makeup appointment at Janna's discretion, but no exception, credit, extension or makeup is guaranteed except as required by law. WHHW messaging channels may be available for some services at any time, but access does not mean continuous monitoring or immediate response. Messages are reviewed during Normal Business Hours and are generally answered within two business days. Messaging is not appropriate for emergencies or urgent medical needs.
11. Laboratories
Laboratory testing, review, pricing, included testing and additional charges vary by clinical service and are stated at checkout or enrollment and in the applicable service agreement. Any additional or repeat testing or WHHW lab-related charge not expressly included must be disclosed and authorized before it is incurred. Laboratories and ordering platforms control their own test-kit cancellation, return, replacement and refund policies unless the applicable service agreement expressly states otherwise.
12. Supplements and financial disclosure
Janna may recommend supplements based on her clinical judgment, product quality and fit with a protocol. Patients purchase supplements directly from WHHW's then-current designated online dispensary or supplier, and WHHW may receive a financial margin or other benefit from those purchases. Recommendations are not intended to be based on compensation. The supplier controls product availability, charges, shipping, returns and refunds under its own terms. WHHW has no other affiliate links, referral payments, paid partnerships or sponsored-product relationships at this time.
13. Testimonials and results
Testimonials, reviews, examples and descriptions of outcomes reflect individual experiences and do not promise that another person will achieve the same or any particular result. WHHW does not offer gifts, discounts, free services or other incentives in exchange for reviews. Before WHHW republishes a review or testimonial that identifies a patient or discusses symptoms, treatment or results, WHHW requires a separate signed authorization describing the approved content and uses.
14. Intellectual property
Unless otherwise stated, WHHW owns or licenses the website design, text, graphics, branding, quizzes, downloads, educational resources and other content. You may use publicly provided resources for your personal, noncommercial use. You may not reproduce, sell, publish, alter, scrape, distribute or use WHHW content to create competing materials without written permission, except as allowed by law.
15. Acceptable use
You may not misuse the website; attempt unauthorized access; interfere with website operation; introduce malware; impersonate another person; submit false information; harvest personal information; violate another person's rights; or use the website for unlawful, abusive or fraudulent purposes.
16. Third-party services and links
The website may rely on or link to third-party platforms, payment processors, laboratories, supplement suppliers, scheduling services, video services and other providers. WHHW is not responsible for a third party's independent content, availability, security, products, policies or actions. Your use of a third-party service may be governed by that provider's terms.
17. Website availability and warranties
To the fullest extent permitted by law, the website and public educational content are provided "as is" and "as available." WHHW does not promise uninterrupted operation, error-free content, compatibility with every device, or that public information will meet your individual needs. Nothing in these Terms limits rights or warranties that cannot legally be limited.
18. Limitation of liability
To the fullest extent permitted by law, WHHW and its owners, workforce and contractors will not be liable for indirect, incidental, special, consequential or punitive damages arising from use of, or inability to use, the public website or free educational content. This provision does not waive liability that cannot legally be waived and does not replace the terms governing paid clinical care.
19. Additional and service-specific terms
WHHW may offer new programs, memberships, subscriptions, events or digital products. Additional terms or a separate written addendum displayed at enrollment or checkout, including price, renewal, cancellation and payment terms, become part of the agreement for that offering. If additional terms conflict with these general Website Terms, the additional terms control for that offering.
No recurring membership is currently presented as an active WHHW offer under these Terms. Any future recurring service will require its own clear enrollment, renewal and cancellation disclosures.
20. Changes and termination
We may update these Terms, modify the website or discontinue public features. Updated Terms will be posted with a revised date. We may restrict access for misuse, security concerns or violation of these Terms.
20A. General provisions
Severability
If any provision of these Terms is held invalid, illegal or unenforceable, that provision will be enforced to the maximum extent permitted and the remaining provisions will remain in effect.
Entire agreement
These Terms, the Medical Disclaimer and any additional terms expressly incorporated at the point of use constitute the entire agreement concerning the public website and free educational content. They do not replace or merge with a signed clinical consent, financial policy, card authorization or service-specific agreement governing paid clinical care.
Assignment
You may not assign or transfer your rights or obligations under these Terms without WHHW's written consent. WHHW may assign these Terms in connection with a lawful reorganization, financing, merger, sale or transfer of all or part of the business, subject to applicable law.
Notices
Notices concerning these Terms may be provided by posting an updated version on the website, by email to an address supplied by the user, or by another reasonable method. Formal notices to WHHW should use the contact information in Section 22 unless a service-specific agreement requires another method.
Force majeure
WHHW is not responsible for delay, interruption or unavailability of the public website or free content caused by events beyond its reasonable control, including utility, telecommunications, hosting, cybersecurity, governmental, labor, public-health, weather or natural-disaster events. This clause does not excuse obligations that cannot lawfully be limited and does not govern paid clinical care unless a service-specific agreement says so.
21. Governing law
These Terms are governed by applicable federal law and the laws of the State of Minnesota, without creating an exclusive venue requirement. This section does not limit consumer rights that apply under another mandatory law.
